List the Academic Excellence concerns made for the degree program, and progress made to address concerns (Dean’s Office has a summary of the concerns).

For last year (AY 2019/2020), provide a concise narrative that includes the program learning outcomes assessed, assessment findings by outcome, and needed actions to maintain success and improve student learning.

For this year (AY 2020/2021), provide a concise narrative on the actions taken, or evaluated previous actions, to maintain success and improve student learning.

List any student success (admissions, retention, advising, graduation etc.) goals identified by the program, or included in the program’s current action plan.

For year (AY 2020/2021), provide a concise narrative how the program implemented data informed change, or evaluated previous actions, to improve student success (admissions, retention, and graduation).

List any assistance that Academic Excellence can provide this coming year to help you accomplish using data to facilitate improvement such as outcome revision, assessment plan revisions, tools and technology to compile assessments.
